{"text":"Data Protection Act 1998: Audit and Research \u2019 - \nYour Persanal Health Information \nWe will record detalls of your medical history, personal life and trealment as part of your routine care. Our staff needs this \ninformation to care for you properly and, like all staff working in the NHS they are bound by a strict code of confidentiality. We \nwill keep this information In your medical case record folder or on computer. \nSharing Your Information . \nMembers of the NHS team locking after you will share your personal health information you will normally meet all of the staff \nand be Intraduced to them by name. As ours is a teaching hospital this may also include those students and trainees in \nmedicine, nursing or other healthcare professions who are looking after you. Students and trainees are also bound by the same \nhigh standards of confidentizlity. Your information will not be available or shared with NHS stafl, trainees or students who are \nnot looking after you directly and do not need to know it. This is called the \"need to know\" principle. \nWe may share some of your personal informaticn with those staff outside the NHS that work closely with us to provide for your \ncare and other needs, for example in social work and education services. The information is restricted {o those items that they \n\"need to know' in arder to help you, We will always ask you for permission to share this information. \nIf you agree, your relatives, friends and carers can also be kept up to date with the progress of your treatment If you do not wish \nany individual to have access to the information in your medical records, you must md:cate this in writing. Forms for this will be \navailable from the medical Records Management Department. \n. The Caldicott Guardian \nThe NHS uses information from patients for a wide range of other purposes. Using information in this way is extremely valuable \nand can improve heaithcare for everybody. We may use it to: protect the health of the general public; chetk that we run.our \nservices efficiently; plan health services for the future; train our staff; carry out medical research. \nWhen we use information in this way itis usually made anonymeous, however we do sametimes use information that identifies \nindividuals and these uses are explained below. This information is always kept confidential and is only used if authorised by \nour Catdicott Guardian. . \\ \nInformation for Clinical Audut \nWe may record same of your personal health information in databases used by doctors and others for clinical audit. This is ane \nway in which we maintain high-quality care. We will check a group of similar cases to your own against agreed standards to see \nwhere improvements can be made. Some of these audits are done over groups of hospitals, over Scotland or even in the whole \nUK. In these cases we make every effort to use anonymous information. We are obliged to action refusal to participate if \npossible. \nInformation for Research \nWe may use some of your personal health informaticn in research. This is when healthcare staff uses information from ttieir \npatients to help them find the causes of disease and the effects of treatment and for planning.new treatments. If the research \ninvolves you personally you will be contacted and asked for your consent. \nLegal and National Requirements \nSometimes we are required by law to pass on information, for example to notify a birth or report certain infectious diseases for \npublic health reasons. \nThe Scottish Executive Health Bepartment also requires information from us to help monitor health services and plan for the \nfuture. We send basic perscnal details of all the patients we care for, along with information about their hospital or clinic \naftendance, to the NHS Information and Statistics Division. \nYour Data Protection Rights \nTheact gives you the right to know how we will use your personal information. This leaflet can only give a short description of \nhew we use information but our Data Protection Officer will be happy to answer any questions you may have. \nYou also have the right to abject to us making.use of your information. You can ask us to change or restrict the way we use your \ninformation and we are oblxged to agree if it is possible to do so0.
