She presented with a history \nof injuring her left Achilles in May of this year following a trip in the bedreom. .She felt instant pain and \ndiscomfort and over time has noticed an increasing general weakness, slapping gait and severe pain around the \nposterior heel area. You kindly sent her for an MRT which confirms a gap to the Achilles tendon suggestive of a \nrupture which is a scarred area with a suggestion of retraction of the tendon by about 4cm. Certainly Mrs \nClelland is walking with a typical gait pattern with a ruptured Achilles; she struggles to tiptoe dnd has very little \npower on plantarflexion. She has a significant amourt of muscle wastage as well. \nWe discussed conservative and surgical options available to her. She feels her activities are severely curtailed \nas well as a general pain. We suggested further intervention in the form of surgery in the form of a tendon \ntransfer and its general risks, benefits and post-op recovery expectations were discussed. She is keen to \npursue and will pass through the pre-assessment clinic to determine her suitability. She is type 2 diabetic and \nher pedal pulses are normal and adequate today. She will be contacted in due course and will be listed for left \nAchilles tendon repair, FHL tendon transfer as an in-patient on the specialist foot list. \nYours sincerely \nMELANIE GREEN
