She is known to us in \nthe past having. undergone surgery to the right mid foot at the Nuffield for fusion of the 2nd MTP joint \nand further down the line tendon transfer for rupture to Achilles to the right side. On review today she \nis very problematic to both 1st MTP joints finding them increasingly problematic with pain, discomfort \nas well as accommadating into footwear. She finds that since her surgery things have escalated \naround the forefoot area. On examination today when weight bearing her hindfoot is well aligned \nrange of movement to the joints are \u2018unrestricted although 1st MTP joints are stiffened. Check x-rays \ndo indicate some degenerate changes consistent with arthritis. We talked through the processes of \nconservative and surgical options to her and sheis keen to go down the line of surgical intervention. \nWe talked through the general risks and benefits and post op recovery expectations involved with \nfusion to both 1st MTP jaints although there is hallux deformities present due to:the arthritic changes. \nShe is keen to pursue she is medically well today although,has angina which\u2019is well controlled and \ntype 2 diabetes. She has lost a tremendous amount of weight over the years which has allowed her \nto reduce her diabetic medication. She is:aware of the general risks of surgery and the prolonged \nrecovery post operatively. She is keen to go down the line of a bilateral case however we have \nmentioned to her the difficulty mobilising, however she will take our advice if unilateral case would \nbe in her best interests. At this stage we will over estimate and we will list her directly for bilateral 1st \nMTP joint fusions as day case.
