She is. a non-smoker. \n~ \nOn examination she struggled to get out of her chair and out of the couch due to back pain.\u201d She \n-has got a slow but normal gait using her stick for support. She was.a.bit tender to palpation of her \nlumbar spine generally. Neurological examination of her lower limbs was normal apart from the fact \nthat | could not elicit ankle reflexes bilaterally. Her knee jerks were normal to brisk bilaterally. She \nhad no altered sensation distally and there was no \u2018obvious motor deficit. She did struggle with \nproximal muscle testing of the hip flexors due to back pain. \nA radlograph of her Iumbar spine organised by her GP shows a new L4\/5 small anterollstheS|s as \n\" well as unilateral Pars defect This is thought to be traumatlc because this not noted on a previous CT \nscan. \n| think this lady\u2019s symptoms are consistent with probably mechanical type back pain in the sense that \nI'think she has some instability of her lumbar spine. She also has neurologlcal symptoms. consistent \nwith stenos:s ' \" \n< \n| have requested an urgent MRI to look for any signs of her having indeed sustained a Pars fracture. \nand whether this is still healing or completely unhealed. | am also very interested in seeing whether \n. she has got pre-existing degenerative disease which could be causing her stenotic picture. \n4 \nI am going to refer her to the spine clinic with this in place because I think she merits review inthe \nmeantime on the basis of her symptoms.
