He reports being breathless on \nwalking any distance, and struggles with house work, he would be MRC grade 4\/5. He reports no \nsleep disturbance or nocturnal dyspnoea. His cough is predominantly non productive, no clear \nprecipitant, no clear diurnal pattemn. He reports no upper airways symptoms, no reflux symptoms. He \ndoes report wheeze. \nHe has a pet cat, he has previously worked as a welder, a joiner and spray painter, and is a current \nsmoker (stopped 1 week ago). Spirometry shows stage 4 COPD. \nCT scan has previously shown cavitating pulmonary nodule that is thought to be inflammatory. \nAspergillus serology was weakly positive with 1gG of 74. This has declined. \n| have requested updated aspergillus serology, and given his nhow non-smoking status he may be \na candidate for long term macrolide therapy, volume reduction therapy or oxygen therapy. | have \nrequested ambulatory oxygen assessment, CT scan to assess whether he may be a candidate for \nvolume reduction therapy and updated full PFTs. | will follow him up in 3 months to see
