He has a good overall understanding of safe GTN use and appropriate use with mode \nof action. He appears to be well up titrated at present given today's blood pressure and heart rate \nreading. He also of note had recent steroid and antibiotic prescription for exacerbation of COPD in \nApril just prior to recent event. He denies any symptoms of ED. \nVITAL SIGNS \nBP 116\/50 mmHg Heart rate 58 bpm and regular. \nMODIFIABLE CORCNARY HEART DISEASE RISK FACTORS: \nSmoking: Current smoker of less than 5 cigarettes per day. This is a reduction from 20. He has fully \nabstained for one week but lapsed and started again. Plans to fully abstain going forward and has \ndeclined SFS input. \nAlcohol: Non drinker. \nDiet: Doesn't meet recommendations. He complains of weight loss and finds it hard to gain weight. \nHe was seen by a dietician one year ago and had been buying build up drinks himself ever since. \nWeight: 60kg. BMI =21. | have referred him with his consent to dietician for further support. \nCholesterol: 5.4 mmol\/l Trig - 1.2, LDL 3.4 on 09\/05\/2023. Newly commenced on Atorvastatin \n80mg following repeat lipids and LFT's. Please consider addition of Ezetimibe 10mg if LDL remains \nelevated above 1.8 mg following repeat Phlebotomy Hub bloods. \nHypertension: No history of hypertension. BP as above. Appears to be well up titrated at present. \nDiabetes: Yes, Type Il. Diet controlled. HbA1c 44 on 09\/05\/2023. \nPhysical Activity: \nStress: When asked about emotional recovery Henry denies any concerns with mood following \nevent but main concern today was stress from finances. Henry has enquired about support but \nnot suitable for any as he earns too much through his pension therefore he feels he needs to \nkeep working in the family company. He feels with better medication control and reduction in his \nbreathlessness he will be able to pursue this and his anxiety levels will resolve.
