He has a PMH of severe \nCOPD, inferier STEMI 2023, ITU admission with diverticular perf 2025. \nDiagnosis: oesophageal spasm and ?pulmonary hypertension for further investigations. \nClinical course: admission bloods showed negative trops, hyperinflated CXR with nil acute signs, \nraised D-dimer 342. Nil oxygen requirement during admission. Given the raised d-dimer was started \non anticoagulation while awaiting CTPA. Given strong PPl and Peptac and analgesia which improved \nsymptoms, chest pain now resolved. CTPA results no definite acute pulmonary embolism but showed \n\u2018strands of low attenuation within the right middle lobe pulmonary arteries are most likely to represent \ncontrast mixing rather than thrombus. A chronic web is the other possibility.\" This was discussed with \nRespiratory who advised patient should have OP V\/Q scan which has been requested on Trak and \nOP Respiratory follow up. He does not need anticoagulation at present. Mr Norwood is how clinically \nwell and can be discharged home with OP follow up as above. \nMedication changes: no changes to long term medications. \nFollow up: OP V\/Q scan requested and OP
